Teacher Nomination Form

WHO MAY NOMINATE:  Teachers may be nominated by anyone: colleagues, administrators, parents, students, family members, or they may self nominate. Nominations are kept confidential unless indicated on the nomination form.

DEADLINE:  The deadline for all nomination submissions is 11:59 PM on Sunday, May 10, 2026.  Once the nomination period has ended, all teachers nominated will be informed and given an E.L.E.V.A.T.E. application.

Please note all nominees must submit a completed application by Sunday, July 12, 2026 and meet the following qualifications to be considered for the award:

1) Spend the majority (over 50%) of the school day in direct instruction to students.

2) Be currently certified to teach Arizona.

3) Have completed at least four full years as a classroom teacher at the same time the application is submitted for consideration.

4) Teach any grade or combination of grades Pre-K through 12 in a school in Pima County.

5) Be a full-time teacher and plan to continue in an active teaching status through the 2026-2027 school year.

If you wish to nominate multiple teachers, please be sure to fill out a separate nomination for each.
